Quality Care Management Coordinator

Utilizes nursing skills and knowledge of resource management and process improvement to coordinate the care for designated patient population. Utilizes a collaborative process that through quality assessment, care planning, implementation of plan, coordination of services, monitoring of efficacy, continuous evaluation and improvement, professional education, consultation, role modeling, community outreach, marketing, research, outcomes development, and use of available resources impact quality, cost-effective outcomes.
REQUIREMENTS
MINIMUM EDUCATION: Registered Nurse with a valid, current Illinois license. Bachelor’s degree or equivalent education/experience.

MINIMUM EXPERIENCE: Minimum three years of clinical experience in an acute care setting with at a minimum of three years’ recent experience in selected patient population. Minimum of two years experience in case management, utilization review, discharge planning process improvement and/or related functions.

PREFERRED CERTIFICATION:Utilization, Case Management, or Process Improvement certification preferred.

SKILLS NEEDED:Excellent communication skills. Computer literacy is required. Proficient with Microsoft Office software – Word, Excel and Access. Demonstrated initiative, ability to work independently and to prioritize work tasks.

WORKING CONDITIONS:The working environment described here is representative of that a healthcare provider encounters performing their essential functions in a hospital environment.
1.Based in an acute care medical center with occasional exposure to excessive noise, fumes and airborne particles.
2.Access to necessary equipment to perform job functions.
3.Fifty percent (59%) of time to be spent in clinical area.
