Job Summary:

Under the direct supervision of the Program Coordinator and consultative supervision of the Occupational Therapy Practice Coordinator, this Occupational Therapist is responsible for providing quality care for patients receiving occupational therapy services, including evaluation, assessment, treatment planning, goal setting, treatment implementation, discharge planning to transition the patient toward maximum independence.

The occupational therapist must document according to professional and facility standards. This Occupational Therapist follows an individualized performance development plan to maintain current and competent in critical areas and support program goals, hospital goals and objectives, hospital functions and philosophy.

Occupational Therapist Essential Functions:

Accepts and responds to Referrals, makes referrals to appropriate resources and educates potential referral sources about the scope of services and the process of initiating services.

Performs complete Evaluations & re-assessments in timely fashion to obtain data necessary for patients of various diagnoses on assigned program.

Establishes Intervention Plan and Goals that are clear, measurable, behavioral, functional, contextually relevant, and appropriate to the client’s needs, desires, and expected outcomes. Incorporates patient and family goals.

Implements treatment intervention through specified purposeful activities and therapeutic methods that are meaningful to the patient. Modifies the process to reflect changes in client status, desires, and response to intervention.

Performs discharge planning to facilitate smooth transition to next level of care: assures patient status is maintained or enhanced through home program instruction, caregiver training, and referral to appropriate follow-up resources.

Completes all required documentation according to program standards, reflecting patient-centered care by relating interventions to established goals & justifying skilled need.

Directs/monitors/ supervises support personnel and co-signs notes as indicated.

Meets program productivity standard while meeting outcome expectations. Participates in data collection & analysis.

Additional Responsibilities:

Participate in supervision of others (i.e. students, interns, & staff orientation) as assigned.

Program development, as appropriate.

Performs duties as assigned.
REQUIREMENTS
Occupational Therapist Minimum Position Qualifications:

Education:

Graduate of an accredited Occupational Therapy professional program or comparable education approved by the National Board of Certification in Occupational Therapy.

Experience:

Be able to perform @ established guidelines for program specific requirements

License / Certification:

Current and valid certificate as a registered occupational therapist issued by the National Board of Certification in Occupational Therapy (NBCOT)

Certification of completion of training in Cardiopulmonary Resuscitation (Class C) which has been completed within the last two years

Preferred Position Qualifications:

Competence in computers and computer applications (word-processing)

Multilingual

AOTA/OTAC membership
