Clinical Liaison-Multiple Valley Wide Clinics

General Summary:
As a member of the clinical team, you would provide clinical leadership and oversight of service delivery; is responsible and accountable for ensuring that consumer’s behavioral health needs are met. Works with the treatment team, including the consumer, their family, community providers and others to identify and implement individualized care and services.

Provides supervision to assigned case managers.


1.Provides clinical and administrative supervision to Case Manager I and/or Case Manager II.


2.Provides clinical oversight to the team as it relates to the delivering of services for an enrolled person, including the assessment and service planning processes.


3.Provides clinical oversight of the person’s care.


4.Works in collaboration with the person and his or her family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.


5.Serves as the point of contact, coordination and communication with other systems where clinical knowledge of the case is important.


6.Ensures the clinical soundness of the assessment and service planning processes; including identifying the need for further or specialty evaluations and signing off on the person’s service plan and annual update.


7.Provides clinical oversight to ensure provision of all covered services identified on the service plan; referrals to community resources as appropriate; and continuity of care between inpatient and outpatient settings, services and supports, as applicable.


8.Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the person and input from the person and relevant others resulting in modification to the service plan as necessary.


9.Ensures the coordination of transfers out-of-area, out-of-state or to an Arizona Long Term Care System (ALTCS) contractor, as applicable.


10.Ensures the development and implementation of transition, discharge and aftercare plans prior to discontinuation of behavioral health services.


REQUIREMENTS
Minimum Qualifications:


Must be able to become credentialed and privileged through ValueOptions credentialing department prior to performing the functions of this position. You must meet the education and work experience combination to be considered for this position.


•Education and Relevant Work Experience:


At least one of the following combinations of education and experience:


•Master’s degree in a behavioral health related field.


•Master’s degree in a non-behavioral health related field and 30 semester hours in behavioral health education and 2 years in behavioral health related work experience.


•Bachelor’s degree in a behavioral health related field and 2 years of behavioral health work experience.

•Bachelor’s degree in a non-behavioral health field and 30 hours of continuing education in behavioral health and 4 years behavioral health work experience.


•AA degree in a behavioral health related field and 4 years behavioral health work experience.


•AA in non-behavioral health related field and 30 semester hours of behavioral health education and 5 years behavioral health work experience.
