Participates in the coordination and direction of care for patients from admission through discharge. The Case Manager must be prepared to address the issues of health assessment and appropriate intervention. The role requires the nurse to have an understanding of financial reimbursements and third party payments. Coordinates and promotes an advanced level of nursing care. REQUIREMENTS
Preferred Qualifications: This position requires a current California RN license with BA/BS (Nursing) or 3 years of recent acute care clinical case management experience. Case Manager experience strongly preferred. Current BLS-C Provider required; Current ACLS Provider preferred. Certification in Case Management preferred. The incumbent will possess superior clinical skills and be capable of high-level assessment and planning for a particular patient aggregate determined by physician and/or diagnosis. Three years of recent acute care experience required. Home healthcare, discharge planning, or prior experience in utilization review are preferred. Knowledge of various reimbursement methodologies preferred. Possesses complex problem-solving skills and the ability to organize and respond quickly to large amounts of sensory stimuli.
