IV Pharmacist
A.Practices pharmacy at all times consistent with implemented IV policies and procedures and in accordance with state and federal law and regulations.
B.Assists the Manager of IV Services in developing IV pharmacy policies and procedures.
C.Properly compounds and dispenses parenteral medications/solutions as outlined in the pharmacy scope of services/policy and procedure manual when necessary.
D.Knowledge in Rescot system.
E.Coordinates and ensures the safe and proper delivery of medications/products of Value Health Care Services, Inc. to LTC facility customers and Home Care patients.
F.Properly handles and disposes of hazardous wastes/medications according to Value Health Care Services, Inc. Infectious Control Policy And Procedure.
G.Maintains patient interactions, adverse drug reactions, and medication error profiles, IV plan of care, patient assessment, and drug monitoring profiles (if necessary)
H.Provides information regarding medications and their usage to patients, nursing and other healthy care professionals involved in the patient’s care.
I.Ensures that ongoing drug therapy is properly monitored and documented and kept current.
J.Review of “Facilities at a Glance” on all assigned facilities.
K.Assists in planning and organizing the activities of the department.
L.Attends pharmacists staff meetings on a regular basis.
M.Communicates with IV pharmacy staff and consultants on ways to improve pharmacy services.
N.Will be expected to assist in and coordinate communication with staff addressing facility problems and follow-up with action to correct in a timely manner.
O.Responsible for correct filling and labeling of all medications issued to facility residents and Home Care patients.
P.Is responsible to maintain records of the transactions of the Pharmacy as required by law and as necessary to maintain adequate control and accountability of all drugs . This may include a system of controls and records for the requisitioning and dispensing of supplies to the facilities.
Q.Promotes positive workplace ethics.
R.Keeps up to date (through continuing education) with current IV trends and developments in pharmacy practice and provides Human Resource Coordinator with CEU’s.
S.Is responsible for maintaining safe work ethics during their shift.
T.Assist Manager of IV Services with the overall IV functioning of the pharmacy during their shift.
U.Active participant in Omnicare Geriatric Care Guidelines and therapeutic interchanges.
V.Participate in Quality Assessment and Improvement (PIP) Committee if necessary.
W.Assume on-call responsibilities as required.
X.Performs other appropriate duties as authorized or delegated by the Manager of IV Services.
Y.Maintain the CONFIDENTIALITY of all pertinent resident care information to assure resident rights are protected.

REQUIREMENTS
A.B.S. Pharm. Or Pharm.D. degree from an accredited school of Pharmacy.
B.Current license in State of Connecticut.
C.Excellent communication, organization and interpersonal skills with patience and ability to be an effective problem solver.
D.Flexibility
E.Experience in a hospital or home infusion therapy, which includes parenteral drug compounding and drug therapy monitoring (preferred but not necessary).
F.Previous working knowledge of computer skills preferred.
G.Physical and sensory requirements (with or without the aid of mechanical devices).
1.Must be able to move intermittently throughout the workday.
2.Must be able to speak and write the English language in a understandable manner.
3.Must be able to cope with the mental and emotional stress of the position.
4.Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
5.Must be in good general health and demonstrate emotional stability.
6.May be required to lift, push, pull, and move office equipment, supplies, etc., in excess of (30) pounds.
