**PER DIEM POSITION**

To ensure that the use of pharmaceutical agents are rational and cost effective.

Co-coordinates the HCP Pharmacy and Therapeutics Committee.
Develops the agenda and participates in the development of treatment algorithms and policy and procedures needed to manage the HCP pharmacy program.
Assists in the development and maintenance of the HCP Preferred Drug List.
Evaluates medications, old and new, that are high cost and/or high risk and/or high volume.
Supervises the Anticoagulation Clinic.
Serves as a drug information resource for the Anticoagulation Clinic nurse.
Processes time cards and conducts evaluations.
Serves as over-time, sick and vacation relief.
Reports to the Regional QI Committee on a quarterly basis.
Supervises the Refill Center.
Serves as a drug information resource for the Refill Center.
Processes time cards and conducts evaluations.
Monitors drug conversion programs.
Assists with prior authorization from health plans as requested by providers.
Participates in disease state management programs (i.e., CHF).
Conducts baseline medication history and educates patients on medication use.
Evaluates pharmaceutical care including untreated indications, improper drug selection, subtherapeutic dosage, overdosage, adverse drug reactions, drug interactions, drug use without indication, and medical conditions resulting from not receiving prescribed medications.
Provides information for PPRC.
Percents generic use.
Percents preferred medication use.
Averages cost per script.
Conducts medication use measurement studies.
Assesses drug treatment and outcomes.
Assesses prescribing and monitoring patterns of medications.
Participates in designing, implementing, and monitoring a therapeutic plan that will produce specific therapeutic outcomes.
Counter-details IPA providers and top utilizers.
Participates on the providers' Speaker's Bureau.
REQUIREMENTS
California Pharm.D. license.
A high school diploma, G.E.D. or equivalent.
Over 3 years and up to and including 5 years of experience.
A masters degree from a four-year college and professional certification requiring formal education beyond a four-year college.
Excellent organizational, interpersonal and decision-making skills.
Knowledge of all Microsoft software.
Capable of working independently.
