Clinical Pharmacist Needed (Work from Home)

To review and provide medication regime appropriateness thru pharmacy claims analyses of HCHR members/institutionalized and non- institutionalized members or other eligible populations that have potential medication challenges. To provide pharmacy resource support to the Care Team who identify/refer members with medication related issues/barriers that preclude members from appropriate self-management of their chronic conditions.

1. Monthly review of medication/ pharmacy claims for early identification of medication regime inappropriateness, non-compliance, non-adherence, and/or to address medication usage issues.
• Drug Utilization Review (DUR)
• Drug Regimen Review (DRR)
• Identifies if other Care Team members need to be consulted/ and or involved.

2.Condition specific intensive review of:

1. Persistent asthma members’ DUR, DRR and provider education to decrease ED utilization/Inpatient admissions’ and improve medication regime and/or costs.
2. Chronic care institutionalized member DUR and DRR to identify poly pharmacy, unnecessary medications or lower cost medication options.
• Knowledgeable of the University of Illinois at Chicago School of Pharmacy Preferred Drug List (PDL). Evaluate the providers’ use/ compliance and preauthorization compliance of medications not on the PDL.
• Understanding and awareness of communication with Comprehensive Neuroscience, Inc (CNSI) specific eligible members and providers.
• Assesses and addresses barriers to medication adherence, compliance and persistence.
• Provides education, information, and direction to members, families and concerned caregivers as prioritized in the care plan regarding medication management issues
• Provides education to providers and office staff
• Works with providers’ to assist in application and qualification for alternative pharmacy funding.
• Works collaboratively to develop and carry out a comprehensive care plan to provide maximum benefit for each member.
• Is a resource to other team members, supports activities
• Meets and/or communicates with other members of the care management team to review program and enrollment numbers
• Identifies, develops and maintains relationships’ with the University Of Illinois at Chicago School of Pharmacy.
• Identifies, develops and maintains relationship with local providers/ high volume providers.
• Identifies, develops and maintains relationship with providers who provide care in long term care institutions (SNF/LTCF)Education/Training:

- Candidate shall be a graduate of a five or six year AACPE accredited School of Pharmacy or equivalent.
- Current unrestricted license as a Registered Pharmacist in the state that they reside
- 3+ years experience as a Registered Pharmacist.

Business Experience –

- Computer proficiency in a Window environment (preferably Word, Excel and Power point)
- Excellent written and verbal communication skills.
- Excellent organizational skills.
- Excellent teaching skills

Knowledge Skills and Abilities:

- Thorough knowledge of clinical pharmacy
- Excellent interpersonal skills.
- Understanding of family and group dynamics
- Familiarity with change behavior techniques
- Skill in establishing rapport with providers, Clinics’, members, and families,
- Ability to communicate and execute work effectively
- Preferably a member of the local community
- Current valid driver’s license and required level of automobile liability insurance coverage.

Working Conditions:
Environment – Independent, home environment with daytime travel to assigned demographic catchment areas and facilities. Must have access to high-speed internet connection (e.g. DSL, Cable) and be able to comply with Work at Home Agent Requirements.
