SCAP Case Manager

The Residential Case Manager is responsible for following through with the daily operations of a 24- hour, seven day-a-week support program servicing adults recovering from serious and prolonged mental illness and co-occurring disorders. As an active member of the client’s treatment team, the SCAP Case Manager is responsible for providing full array of case management services, which includes but is not limited to clinical assessment, reporting and documentation, as well as rehabilitation- and recovery-oriented treatment activities.

•Provides daily assessment of individual client needs and works with the rest of the team to develop treatment interventions as it pertains to each individual client.
•Provides individual and group supportive counseling in the areas of problem solving, coping skills and interpersonal interactions.
•Provides skills teaching and “hands on” training in areas such as medication independence, housekeeping skills, ADL’s, meal planning, food preparation, grocery shopping, nutrition planning, budgeting and use of community resources
•Advocates for clients needs as relevant to medical providers, external treatment providers, vocational services, housing authorities, landlords, and other community providers as necessary.
•Outreach and engagement with clients in various community settings, including clients’ home, shelters, rest homes, soup kitchens, etc.
•Supervises clients in their self administration of medication in accordance with DMHAS regulations and Gilead’s policies and procedures.
•Provides assistance with arranging transportation to treatment and other community supports appointments and transportation to essential appointments when no other arrangement is timely or/and practical and such transport can be safely achieved (using personal vehicles may be required)
REQUIREMENTS
Education: BA Degree in Human Services required
Experience and skills: Minimum 1-year exp in community mental health preferred
