In 1994, The Institute of Living, a psychiatric hospital founded in 1822, became the Mental Health Division of Hartford Hospital.

Hours: Two P/T positions – each 20 hours; Hours vary w/ some flexibility Monday thru Friday (Days)Telemanagement /Coordination of care of CHF outpatients to provide monitoring, CHF education and emotional support to both patient and family. Enroll/recruit new patients. This position requires that the applicant be self-directed, highly motivated and resourceful w/ excellent clinical assessment skills and excellent verbal and written communication skills. Data collection on CHF population.As a teaching hospital, Hartford Hospital is a primary affiliate of the University of Connecticut School of Medicine and Dentistry for both graduate and undergraduate medical education.

REQUIREMENTS
RN CT license and BSN required. Minimum of 2 years cardiology or critical care experience. Bilingual/Spanish preferred. Basic PC/Windows computer skills.
