Occupationa Therapist (OTR) for Colorado Springs CO

Pikes Peak Care Center Therapy Team seeks an Occupational Therapist (OTR) experienced in Skilled Nursing/Long Term Care for our 210-Bed dually certified Skilled Nursing Facility in Colorado Springs, CO. In general, the OTR provides and directs Occupational Therapy services to patients including assessment, treatment, program planning and implementation, related documentation and communication, functioning under physician’s orders. The Occupational Therapist is responsible for:

1. Providing Occupational Therapy services to patients and initiates referrals when appropriate and in accordance with the Therapy Department procedures.

2. Conducting screenings of patients at regular intervals to determine need for intervention/treatment.

3. Evaluating patients to obtain data necessary for treatment planning and implementation. Conducting specialized evaluations as indicated.

4. Developing treatment plans by identifying a problem list, including short- and long-term goals and methods to achieve identified goals.

5. Implementing and conducting treatment as outlined in treatment plan.

6. Monitoring patient’s response to intervention and modifying treatment as indicated to attain goals in accordance with physician orders.

7. Developing appropriate home or community programming to maintain and enhance the performance of the patient in their own environment.

8. Interpreting and communicating evaluation findings to patients, family, and other team members.
REQUIREMENTS
This position requires a Bachelor of Science Degree in Occupational Therapy from an accredited program. A NBCOT certificate is required. Most importantly, the OTR will have a sincere commitment to the health and well-being of our Senior patients.
