The Summit at Plantsville Health Care Center of Southington is a 150-bed skilled nursing and subacute rehabilitation facility. Managed by Southington-based Athena Health Care Systems, The Summit is dedicated to providing the highest level of care to those with even the most medically complex cases.

In this exciting role, the selected candidate will be responsible for monitoring the health and wellness of residents, you will perform medication passes, conduct assessments, coordinate services with outside care providers and work closely with frontline staff.

We currently have two positions available:

Position: LPN
Hours: 24
Shift: 3-11pm
Sign On Bonus: $1200

Position: LPN
Hours: 40
Shift: 3-11pm
Sign On Bonus: $2000

REQUIREMENTS
Must be a graduate or an approved LPN/LVN program

Must be CPR certified

A minimum one (1) year experience in a hospital, long term care facility or other related health care facility is preferable

Training in rehabilitative and restorative nursing practices
