This position is integral to the Banner Health System in order to integrate services with acute and long-term services. The RN hospice care coordinator organizes, gathers, and records information that is necessary to initiate the plan of care for all referrals (i.e Hospice and Home Medical Equipment) in accordance with State and Federal Regulations, JCAHO Standards, and per policy for program admission criteria.
