Baldwin Case Manager

RESPONSIBILITIES:
•Assess and evaluate client needs
•Complete necessary case documentation on each client
•Conduct client orientation. Collect intake data, perform client needs assessment and make recommendations to clinical team regarding treatment needs•Schedule, conduct and document psycho educational group sessions addressing client personal issues and program performance
•Maintain client files and comply with Agency/ Program/ DCF policies regarding confidential information and mandated reporting requirements
•Complete all forms, progress reports, and closing summaries on all assigned clients
•Coordinate discharges with referral agency, in collaboration with Senior Case Manager and Program Manager
•Monitor clients behavior and implement program behavior management procedures as needed
•Assist clients in working on program goals and provide client services in area of growth and responsibility, education, vocational, and needs related to family and friends
•Develop and maintain professional boundaries and relations with clients
•Serve as positive role model following program guidelines for professional conduct and code of ethics
•Provide programmatic and statistical reports as required to Senior Case Manager•Attend and participate in agency meetings, staff meetings, supervision and on-going required trainings
•Must be familiar with universal precautions and related issues including risk of communicable disease•Assist in the transition of clients to a more independent living
REQUIREMENTS
EDUCATION: BS/BA degree in social work or related field, Masters Degree preferredEXPERIENCE AND SKILLS: One year experience in the human services field plus one year of vocational experience and knowledge of community mental health systems and adolescent male issues. Trauma-informed knowledge preferred. Experience working with mental ill in residential setting and counseling background preferred. Computer skills required. Must be able to work weekends. Must have and maintain valid CT driver’s license.
