Transition to Adult Coordinator

Duties:
The Transition to Adult Coordinator would provide consulting and assistance to Child and Family Teams with young adults, ages 16-18 who are beginning the transition to adulthood. This may include those who will transition to SMI and those who will remain at JFCS in General Mental Health. Also, the coordinator would identify specific resources to assist this population in becoming successful after leaving the children's system and assist teams and clinicians to secure resources necessary for those young adults ages 18-21. Much of the client contact will occur in the field.
Requirements:
Bachelor's degree in Social Work, Sociology, Psychology or related field and a minimum of 1-2 years experience with weekly facilitation of Child and Family Teams required.
