Targeted Case Managers (Adult and Children)

Position Overview
The Targeted Case Manager provides supportive services including outreach, monitoring, assessment and service planning, support network building, and instruction in use of community resources to consumers with mental illness.Essential Job Responsibilities/Requirements
-Provide individualized case management services based on psychosocial psychiatric rehabilitation approach including community/home visits, assessment of strengths and needs, recovery-oriented engagement and cooperative partnership development, aggressive outreach, commnity resource linkage/acquisition and individualized stage of change-based service planning at community locations such as the home, street, or school as needed and clinically appropriate.-Assist consumers in obtaining and maintaining skills and resources defined in the service plan that will provide for their basic living needs such as housing, food, education, medical care, financial entitlements and social support.-Maintain percentage of allocated hours in direct service activity.-Participate in rotating after hours on-call services as required.-Complete required documentation.-Maintain valid driver's license and safe driving record.Education/Experience/Training
A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, education; orA registered nurse; orA high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years experience in direct contact with mental health consumers; orA high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.Affiliated with Mennonite Health Services
