About the Opportunity
Get to know our residents and their families to give them the kind of psychosocial support they need to cope with what is often a very difficult time for them. Train staff to offer the care needed from assessment at admission through change in condition to potential discharge.
Responsibilities:
Ensure that residents receive psychosocial assessments at admission, upon change in condition and/or annually; direct and guide staff in conducting assessments and interventions
Provide direct psychosocial intervention; create, review and update care plan and progress notes
Assist resident's family in coping with placement in a skilled nursing facility, physical illness, disabilities of the resident and the grieving process
At admission, work with the resident, family and team members to outline goals of stay, the plan to meet those goals and discharge as appropriate
Perform management functions such as hiring, evaluating and disciplining employees
Conduct in-service programs to educate staff regarding psychosocial issues and resident rights
Conduct job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards, and applicable state/federal laws
Qualifications
The Director will be knowledgeable in psychosocial practices applicable in a long-term care environment and skilled at directing and motivating the staff. Will work cooperatively as a member of a team. The ability to communicate effectively with residents, their families and at all levels of the organization is important. Must maintain confidentiality. Requires a MSSW degree, state licensing, and one year of experience in a long-term care environment.
