Case Manager

Chicago House and Social Service Agency, a Non-profit housing/social service agency for individuals and families with HIV/AIDS is seeking a Case Manager to provide full range of case management services for individuals with HIV/AIDS
Some of the duties of the Case Manager include:
* Collaborate with Staff to assure quality care and to identify
appropriate services the client.
* Manage an assigned caseload of clients with HIV/AIDS requiring
comprehensive DHS,(Department of Human Services) DRS (Division of
Rehab), Home Services Program, Ryan White and Prevention Case
Management.
* Monitor the work of DRS funded contractual services providers.
* To comply and adhere to the protocol and the Case Management
Standards of DHS,DRS, Home Services Program.
* Provide a full range of case management services: referrals,
intakes, assessment, service planning, service linkage, discharge
planning, etc.
* Work with appropriate professionals as an advocate for clients in
need of benefits and entitlements and any other unidentified
services.
* Develop and maintain effective working relationships with other
service providers, network providers and outside consultants.
* Attend all required trainings and meetings through the
Northeastern Illinois Case Management Cooperative and Chicago
House.
REQUIREMENTS
* Bachelor's Degree in Social Work or Master Degree in Social Work
preferred.
* Bachelor's Degree in a Human Service Field with a minimum of 5
years of case management experience.
* Familiarity with HIV/AIDS, Substance Abuse and Mental Health.
* Excellent oral and written communication skills.
* Ability to work cooperatively as part of a team.
* Previous case management experience preferred.
* Bilingual/bi-cultural preferred.
