CASE MANAGERCovenant House Florida serves homeless, runaway, and at-risk youth under 21 including teen parents and their babies. Every day we reach nearly 175 kids in Fort Lauderdale and Orlando via street outreach, crisis shelters, transitional housing projects, and aftercare, making Covenant House Florida one of the largest private agencies serving homeless youth in the state of Florida.
We are currently seeking both Full Time and Per Diem (on call, as needed) Case Managers to work at our 104-bed residential facility located near Fort Lauderdale beach.
SCHEDULE: The Case Manager schedules vary but usually include working both daytime(8-4)and evening shifts (3-11), including weekends. Overnight shifts from 10:30pm-8:30am are also available.PURPOSE OF THE POSITION: Case Managers provide direct supervision and case management services to homeless and runaway youth residing in Covenant House Florida’s residential crisis shelter.BENEFITS: Full time employees receive full benefits including health, dental insurance, pension, generous time off, and tuition reimbursement for employees who have been with the agency for one year. Full time employees are also eligible for annual salary increases.
Full Time: $12.98 per hour will full benefits
Per Diem (work on call, as needed): $10.00 per hourESSENTIAL DUTIES AND RESPONSIBILITIES:Crisis Prevention and Intervention: Supervise residents, maintain order, and exercise appropriate judgment at all times (i.e. at meal times, on the residential floors, in common areas, on recreations, and during escorts). Assist youth in emergencies as directed by supervisor.Sanctuary and Physical Plant: Maintain a clean and safe environment. Assist and supervise youth in general cleanliness of all residential living and common areas. Ensure that work areas are clean and organized before, during, and at end of shift.Case Management: Greet all residents unconditionally and aid them in assimilating to the CHF environment. Establish rapport and trust with residents. Ensure that services rendered to youth are in a culturally competent manner according to CHF Mission, Principles, and Core Values. Familiarize and assist residents with the assessment process. Gather and document all necessary information for plan development process. Guide and advocate for the residents in planning future goals and setting attainable objectives. Provide support, encouragement, and accountability to the residents in their daily activities so they fulfill their plans. Prepare residents for discharge so they can maintain and build upon their plan achievements.Communication: Communicate pertinent information with staff and youth throughout each shift. Communicate respect, mutual support, honesty, and caring to all youth and staff. Take an active part in shift transition and provide vital information about residents and referrals. By the end of each shift, complete all documentation neatly and accurately (i.e. case records, incident reports, abuse reports). Provide on-line supervision and support to volunteers and student interns who assist in implementation of services at the Residential Crisis Shelter.Referrals: Assist residents and families with the necessary community resources/referrals to advance youths’ plans, including escorting youths to the appointment as appropriate. Make referrals to other agencies and in-house projects, as well as following up with these referrals.
