RN for Onsite D/C planning case management care coordination

INSPIRIS is looking for an RN Care Coordinator to assist in healthcare coordination, discharge planning, continued stay review, provide assistance in placement and make recommendations for alternatives in care, so we can streamline the care of our patients and safely decrease their length of stay.

Diversion is a primary element of how employee and INSPIRIS is measured in addition to length of stay reductions in both acute and post-acute settings.

This is an autonomous position that works closely building external relationships with hospital-based physicians, CIGNA case management staff, hospital staff; same for post acute facilities.
REQUIREMENTS
Arizona RN license in good standing. We are seking experienced RNs with 1-2 yrs Case Management or 3-5 yrs in a speciality field. DC planning and Managed Care experience preferred.
