Director of Case Management - 3176 - 00031

Job Summary: Directs the staff that coordinates the care and service of an assigned patient population, during an episode of illness promoting and monitoring effective utilization of health care resources. Assumes a leadership role in facilitating the care of patients during a disease/catastrophic episode by managing, coordinating and monitoring resources to achieve optimal clinical, utilization, service and financial outcomes.
Qualifications Position Requirements Education: BS Degree and RN license required. Experience: 3 years case management experience required. Previous Quality experience preferred. Licensure/Certification: CCM or CHPQ preferred
