Looking for RN Clinical Supervisor!! 20 hrs/week(starting out/may be full time shortly)($16-$20/hr)

POSITION TITLE:CLINICAL SUPERVISOR


POSITION SUMMARY:A Registered Nurse who is directly responsible for clinical activities of the field staff. With team member, plan, implement and evaluates patients Plans of care for appropriateness to individual patients needs. Plans for and assists in conducting team conferences. Shares in the responsibility of assessing clinical case management activities. A nurse supervisor or his or her registered nurse designee shall be available on the premises or immediately accessible by telecommunications during operating hours when patients are receiving services.

POSITION RESPONSIBILITIES:

1.Assures that the initial in-home evaluation of any patients requiring the service of a registered nurse, licensed practical nurse, home health aide or other nursing support staff is performed by a registered nurse.
2.Enforces and demonstrates 100% compliance with all Federal, State, and JCAHO regulations, reimbursement criteria as well as Maxim policies and procedures at all times.
3.Assess and supervise the delivery of nursing services.
4.Prepare and coordinate clinical notes and progress notes on nursing services delivered.
5.Provide in home supervision of nursing services delivered by licensed nurses, home health aides and other nursing support staff while the employees is present and delivering services.
6.Report changes in the patient’s medical or mental condition to the attending physician and to the Director of Patient Care Services.
7.Facilitate and promote patient and family or significant other in education to assist and promote return of optimal health status related to specific psychosocial and physical needs.
8.Care plans are implemented and updated as necessary during supervisory visit.
Initiates the patient's plan of care in accordance with nursing needs specified in the approved plan of treatment. Re-evaluation will occur at least every 60 days.
9.Participate in the continuing review of service, and participation in the performance improvement program.
10.Plans for and assist in conducting multidisciplinary meetings on a biweekly basis or as directed.
