Must be able to work independently with discretion and judgment to achieve program objectives. Participates in weekly supervision. May work directly with Program Coordinator, Team Lead, assigned Therapist and Psychiatrist on a project-by-project basis. Assesses consumers in accordance with established policies and procedures and case management protocols. Engages in Family Centered, strengths-based service planning and coordination. Participate in the development of individual treatment plans for consumers. Meets with clinical team to review assessments and treatment plans, and to develop plans of service for consumers. Develops and maintains therapeutic relationships with consumers and significant support systems. Provides information, education and guidance to consumers and support system members concerning issues of treatment planning, advocacy, and coordination of care. Monitors mental status of consumers and response to treatment plans through face to face contact, and updates treatment team (therapist/PBHP) as needed. Meets frequently with consumer in their natural setting. Coordinates and monitors services for consumers as outlined on the individual treatment plan developed by the therapist/PBHP. Works, as a member of a clinical team, to assess consumers’ needs, strengths and goals. Identifies funding for services. Assists assigned therapist/PBHP in the development of treatment and service plans. Notifies Therapist/PBHP, Team Lead and in an emergency agency Clinical Supervisor, of actual or potential problems with consumers; i.e., non-compliance, decompensation, and crisis. Meets frequently with clinical teams and provides updates on treatment progress/barriers so that the assigned therapist/PBHP can review and modify the service plans as needed. Coordinates discharge plan with assigned therapist/PBHP and consumer upon conclusion of treatment. Provides information to consumers and family members concerning community resources and linkages to other services. Assists consumers and families in securing appointments with outside services and agencies, as needed. Arranges transportation for consumers and family members for community services as directed by the therapist/PBHP and identified on the Individual Treatment Plan. (Case managers may also have to provide transportation of clients in their own vehicles if therapeutically needed). On-call coverage will be rotated through case managers to cover any wellness checks or crisis interventions on the weekends. Maintains accurate, thorough and current documentation of contact with consumers and timely service authorization logs. Ensures complete and accurate progress notes of all serious incidents and changes in behavior. Ensures confidentiality of verbal and written consumer information. Other duties as assigned.
REQUIREMENTS Bachelor’s degree in a social science with at least 1 yr. of case management experience.
Bilingual (English/Spanish) speaking preferred.
Knowledge of community based behavioral health services preferred.
Excellent written and verbal skill, the ability to problem solve and think outside the box.
The ability to advocate for families and their needs.
