Summary/Position Objectives:
Under the direction of the Long Term Care Supervisor, the case manager will ensure that appropriate and cost effective medical, medically-related social services, and behavioral health services are identified, planned, obtained.

Responsibilities:
Essential Functions:
Serve as interdisciplinary team member in management of identified members, performing medical/ social - status assessments and determining appropriate medical / social services.
Provides a continuum of service options that support the expectations and agreements established through the care plan process.
Integrate access to services available throughout the community.
Act as liaison between PCP and member with problems that are identified which potentially could interrupt care. May require travel to members homes, physician offices, pharmacies, etc.
Provide information and/or education to providers and health plan staff on identified issues that result in high utilization of services.
Provides coordination across all facets of the service system in order to maximize the efficient use of resources and minimize any negative impact to the member.
Maintain documentation as required according to health plan policy and procedures.
Prepare reports for AHCCCS, health plan and department as needed.
Secondary Functions:
Represent health plan at various meetings with community agencies that provide services to members.
Attend routine staff meetings and in-services.
Works in a manner that is not disruptive to peers, supervisors and/or subordinates.
Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
Must be available and willing to work such days and hours as the employer determines are necessary or desirable to meet its business needs.
Must be available and willing to travel to such locations and with such frequency as the employer determines is necessary or desirable to meet its business needs. (If travel required.)
REQUIREMENTS
Knowledge and Skills:
Requires considerable logic, ability to analyze problems and determine a course of action.
Must be able to communicate, write, advise and counsel on complex matters.
Requires tact, insight and leadership skills.
Must be proficient in MS software applications.
Valid drivers license required.


Education and Work Experience:
Bachelor’s degree or relevant experience in lieu of degree.
2-5 years experience in Public Health, Home Health, Community Health, or other healthcare area providing services to clients, including coordinating with community agencies.
