Case Manager

Department: Case ManagementJob Description:
Supports physician in provision of patient care with accountability for designated patient caseload. Facilitates pre-certification and payor authorization processes; facilitates collaborative management of patient care across continuum, intervening as necessary. Plans to meet patient needs to ensure high quality of patient care, reduced length of stay, efficient utilization of resources and appropriate reimbursement of care and services. Applies process improvement methodologies in evaluating outcomes of care. Supports Clinical Documentation Improvement Program (CDI) by performing medical record reviews, identifying correct DRG designation, ensuring documentation accurately reflects acuity. Serves as clinical resource for medical records coders and coordinate communications with physicians.
REQUIREMENTS
Education Required: Associate degree in nursing required with Bachelor's degree in nursing preferred. Currently licensed as RN by State of Alabama required. Ability to operate various office machines, computer and software packages required.Experience Required: Three to four years’ experience with one year recent clinical experience in specialized area required.
