Full Job Title: Care Coordinator - Case Management - Part Time - Days - 9663 - 34222The RN Case Manager works with physicians and other members of the multidisciplinary team to develop a plan for each patient from admission to discharge. Job Summary: Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages of patients served as per Scope of Service. Promotes internal and external customer service initiatives. Utilizes critical thinking skills to meet needs of individual patients. Demonstrates ability to perform technical skills and procedures. Documents all patient care. Participate in Performance Improvement activities as assigned. Works with business office to manage payment issues of third party payers. Communicates with third party payers to review patients' care. Acts as a liaison with patients, physicians, and Program Coordinator to keep them informed of care issues, such as discharge plans, insurance reviews, etc. Actively works with treatment team to assist patient and family with discharge plans.
Qualifications Required Job Qualifications include: Graduate of an accredited school of professional nursing or Masters in Social Work degree required. CCM preferred Current BLS certification. Minimum 5 years of clinical nursing experience. Knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules preferred. Customer service abilities including effective listening skills. Critical thinking skills, decisive judgment and the ability to work with minimal supervision meeting firm deadlines. Assertive and persuasive communication skills, action oriented.
