Behavioral Health Coordinator

The Behavioral Health Coordinator performs professional oversight of the management of behavioral health services as well as performs case management activities. Work often requires development of recommendations and independent actions consistent with the policies, directives and regulations from AHCCCS and other State Regulators.

Performs assessment of members seeking mental health/substance abuse case management and stratifies high risk members in order to be in Case Management.

Identifies appropriate agency referral as necessary and accurately document information into the appropriate business application program.

Assists member in seeking appropriate referrals to community resources.

Takes action to assist member in achieving identified goals as appropriate through coordination of care and follow-up.

Clarifies client expectations.

Identifies medication and physician issues and handles appropriately based on PHP/Abrazo guidelines.

Participates and seeks educational programs to increase knowledge of behavioral health issues.

Performs out-reach and follow-up telephone calls to identify behavioral health or pain management issues and work toward resolution.

Assist with the review of behavioral health utilization data to identify trends, and make appropriate recommendations.

Assists in the development of in-service training and other programs that includes orientation of new staff.

Maintains records in compliance with County, State and Federal requirements.

Assists with any AHCCCS or Abrazo corrective action plans, PIPs or QIPs.

Acts as a liaison for behavioral health care providers.
REQUIREMENTS
Active AZ Licensed Psych Nurse or Social Worker, unrestricted license.

One to two years of mental health, medical/health care or social work experience, preferable in a community health setting. Medical record review/data collection experience preferable.

Minimum of 2 years working in the behavioral health arena. Bilingual

Knowledgeable in Behavioral Health and programs provided to Medicaid/Medicare population. Good computer skills, knowledge of medical terminology, good interpersonal communication and customer skills. Ability to analyze and create reports, as necessary.
