Only 30 min north of Madison - Our facility is located one block from the hospital and has a strong relationship with this referral source. Aegis Therapies is the in-house provider of rehab services here; our parent company owns the facility, so there is not threat of a lost contract. At 99 beds, we have an out patient unit, a wellness program and the facility is broken into four units: ACU, AACU, Medicare Wing & LTC. Imagine the diversity this could bring to your caseload! The facility has been updated in the last few years with new furniture, carpeting and a new Medicare wing. Our gym is comprised of two large rooms with Nautilus equipment, modality equipment (Ultrasound, e-stim & diathermy …) and we even have a rehab aide to assist with transporting. At present, our team consists of 6 members and we are looking to expand. Their productivity hovers around 75% (plenty of time to get your paperwork done as well as develop additional programs), and we typically see about 2/3s Med A. REQUIREMENTS
Physical Therapy
Performs evaluations, develops and implements effective patient treatment plans to restore, compensate or adapt for loss of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing the facility and their discipline.

Qualifications
•Graduate from an accredited program for Physical Therapy.
•Must possess and maintain a current license in the state of practice as required.
•Must possess problem-solving skills of the type and at a level necessary to accomplish the job.
•Must demonstrate good time management and organizational skills.
•Must be able to communicate in English, both verbally and in writing.
•Must be able to relate professionally, positively, and cooperatively with patients, patient’s family members and facility staff.
•Must be capable of maintaining regular attendance.
•Must meet all local health regulations, pass post-offer drug test, and post-offer employment physical exam if required.
•Must be capable of performing essential job functions of this job, with or without reasonable accommodations.
•Must have own transportation to travel between sites as needed.
•Must possess computer skills of the type and at a level necessary to accomplish specific job duties and assignments.
•Must meet Clinical Competency requirements upon hire and as needed.
