Your role would be pivotal and some of your responsibilities would include:




Assessing telephonically the needs of our patients/members, developing their treatment plans, monitoring their progress and documenting all procedures




Participating in rounds with medical director and doctors




Educating patients on how to take the best care of themselves




Partaking in in-house training, meetings and other administrative duties


Job Requirements:

You must be a RN in the state of Wisconsin with a current and unrestricted license and a Bachelor's degree. Prior clinical experience is also required. Excellent computer, communication and assessment skills are essential. Candidates with case management experience and a background in specialty education are highly encouraged to apply.
