Reports to Director of MR/DD Waiver Program Responsibilities:
Administers assessments utilizing specialized software to complete annual evaluations for development of a comprehensive plan of services and supports for persons in the MR/DD Waiver program Educates family and/or consumer on person centered model as it relates to assessed needs/wants Conducts reassessments when necessary Consults on cases when appropriate Trains a variety of stakeholders Travel required

REQUIREMENTS
Requirements:
At least three year’s experience with MR/DD population Strong verbal and written communication skills Ability to resolve problems in a non-threatening manner Experience with the WV Public Sector Behavioral Health System preferred Training in Person-Centered Planning a plus Proficient computer skills
Education:
Master’s Degree in related field preferred or Bachelor’s degree with relevant experience
