RN Admissions Nurse HOSPICE INSPIRIS full time M-F

INSPIRIS and HOSPICE INSPIRIS are premier providers long-term, post-acute and hospice and palliative care services. We are unique. We bring our quality care services and unique sense of caring to the patient’s residence: their home, skilled nursing center assisted living center or group home.
We are currently seeking an Admissions RN. The RN Admissions Nurse works in a team approach to conduct initial assessments and establish the initial plan of care for hospice patients. Provides initial training and education to family caregivers. Provides information about services to potential hospice patients. Admits patients to hospice services.
Our ideal candidate is available from approx 11a-7p Monday thru Friday to open cases in Phoenix and surrounding cities.

REQUIREMENTS
RN degree in good standing, experience with hospice or home health case management.
