As a Hospice Admissions Nurse, you would help our newest members join the hospice program while enjoying a dynamic team, flexibility and all the perks that come with working for an industry leader.



Primary responsibilities include:


Providing office assistance via phone admissions and referrals


Processing referrals for new patients


Visiting patients in their homes/facilities to evaluate their care/health status


Working closely with patients' families to explain hospice services


Job Requirements:

The chosen candidate must be an RN with a current, unrestricted license and possess strong acute clinical experience. A background in hospice and/or admissions is highly preferred, and home health knowledge is valuable. Computer skills are a must. Certified Case Managers are encouraged to apply.
