Program: Home Health and Hospice

Status: Full-time (32-40 hours/week)

Description:
Office based position for an experienced Registered Nurse

Working from our Sunnyvale office, develops homecare and/or hospice referrals for those patients needing home care. Communicates Pathways' home health and hospice patient criteria and agency admission policies to the professional community. Gives information regarding Pathways' programs and community resources to the community at large.

REQUIREMENTS
Licensed as a Registered Nurse in the State of California. Minimum of one year experience in an acute care/sub acute environment with adult med/surg or oncology patients. Home care or hospice experience preferred. Demonstrated strong telephone interviewing and assessment skills. Strong organizational skills with the ability to work independently. Demonstrated strong customer service skills. Must have basic computer skills.
